I grant to the Positive Impact for Life Summer Reading for Success Program, its representatives and employees the right to take photographs of my child. I authorize the Positive Impact for Life Summer Reading for Success Program, its assigns and transferees to copyright, use and publish the same in print and/or electronically. I agree that the Positive Impact for Life Summer Reading for Success Program may use such photographs of me with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and Web content. I have read and understand the above.

To ensure the safety of myself and others, I agree to follow the rules of the program and the instructions given by the Positive Impact for Life Summer Reading Program staff. I will arrive on time to all sessions. I will tell Positive Impact for Life Summer Reading for Success Program staff if I cannot attend one of the sessions. If I have more than 3 unexcused absences, I will risk expulsion from the program. I have read and understand the above.
